Job summary

Horizon Care Ltd in Coventry is seeking a Youth Support Worker to help young people transition to independent living. You will provide emotional and practical support and work to develop personalised support plans.

The role requires safeguarding knowledge, resilience, and strong communication. Flexibility to work shifts including evenings, weekends, and sleep-ins, plus a driving license and access to a vehicle.

Qualifications

• Experience working with vulnerable young people, ideally care leavers.
• Safeguarding knowledge and risk assessment skills.
• Ability to be resilient and empathetic.
• Excellent communication and problem-solving abilities.
• Flexibility to work evenings, weekends and sleep-ins.
• Driving license and access to a vehicle.

Responsibilities

• Help young people access education, training, and employment.
• Manage properties and liaise with other professionals.

As a Youth Support Worker you will be helping young people transition to independent living, duties include:

• Provide emotional and practical support , including out-of-hours on-call.
• Develop and review support plans, ensuring safeguarding and risk management.
• Help young people access education, training, and employment.
• Manage properties and liaise with other professionals.
• Support our young people to develop independent living skills to prepare them to live fully independently .
