Job summary

Early Break in Salford is seeking a Young People’s Specialist Substance Use Worker to provide one-to-one, face-to-face support to young people and their families, with flexibility for occasional evenings or weekends.

You will complete assessments, develop SMART care plans, deliver interventions (including substitute prescribing where appropriate), work in community settings, collaborate with partner agencies, and maintain accurate records in line with data protection policies.

Responsibilities

  • Deliver 1:1, face-to-face work with young clients and their parents/carers as appropriate, with sessions potentially in evenings or at weekends.
  • Complete a full assessment of each client’s needs and develop, monitor and review SMART care plans.
  • Offer structured treatment interventions, including substitute prescribing when required and appropriate.
  • Make alternative therapy interventions part of client care planning and upskill to offer these when relevant.
  • Work in community-based settings to engage and deliver services to potential clients.
  • Liaise with a range of agencies to secure provision meeting treatment needs identified through referrals.
  • Offer drop-ins as required and/or appropriate in relevant young people settings.
  • Contribute to multi-agency meetings as lead worker/advocate for the young client.
  • Support young clients into inpatient treatment when available/as appropriate, with after-care planning as needed.
  • Adhere to safeguarding policies and Data Protection requirements, reporting and recording client information accurately.
  • Maintain and update knowledge through training provided by Early Break and partner agencies.
  • Contribute to fundraising and charitable activities.
