Job summary

Youth Connect South West (YCSW) in Bath is seeking a dedicated Key Worker to join our Alternative Education Team. You will provide intensive support to young people aged 16-25 with SEND, helping them access educational opportunities and prepare for adulthood.

You will develop personalised action plans, coordinate with professionals, and support young people in their educational and personal development journeys.

Qualifications

• Experience supporting young people with SEND.
• Ability to develop personalised action plans.
• Willingness to travel to homes across B&NES.

Responsibilities

• Provide intensive support to young people with SEND.
• Develop personalised action plans focusing on strengths, needs, and aspirations.
• Coordinate with professionals to deliver holistic support.
• Attend and contribute to multi-agency meetings.
• Maintain accurate records and safeguarding compliance.
