Job Title: Young Persons Support Worker - 16-25 year olds service
Location: Chesterfield, Derbyshire
Contract: Full-time, temporary to permanent
Salary: £17-18 per hour

We are looking for a dedicated Young Persons Support Worker to join our team, providing person-centred support to young people living in supported accommodation.

About the Role

As a Young Persons Support Worker, you will:

• Provide tailored, one-to-one and group support to young people, helping them to build resilience, develop life skills, and move towards independent living.
• Support young people to understand safeguarding and safety, empowering them to make positive choices.
• Build strong, trusting relationships using a trauma-informed approach.
• Coordinate support plans and work closely with colleagues, housing providers, and external agencies to meet young people\'s needs.
• Maintain accurate records in line with GDPR and safeguarding policies.
• Act as first point of contact for your caseload, responding to incidents, emergencies, and safeguarding concerns appropriately.
• Encourage young people to co-design their own support plans and take an active role in shaping their future.

About You

Looking for a Young Persons Support Worker who is:

• Experienced in working with young people or vulnerable groups (experience in supported housing, social care, or youth work is desirable).
• Adaptable, creative, and engaging in their approach to support.
• Able to work independently, including lone working and out-of-hours shifts.
• Confident in managing safeguarding concerns and responding to crises.
• Organised and able to maintain clear records and reports.
• Passionate about empowering young people to reach their potential.
• Able to commute around Chesterfield.
