Young Persons Engagement Worker

An exciting opportunity has arisen to join our small team of Young Persons Engagement Workers to work across the A2i team and CAMH's services in Portsmouth.

The successful applicant will support young people aged between 16-25 who require additional support to aid engagement with Adult Mental Health services or accessing support networks in their local community upon discharge from CAMHS.

They will work as a member of a multi-disciplinary/multi agency team and demonstrate a good level of specialist skills and experience in supporting young people with mental health needs alongside multi-complex and at times long-standing difficulties.

The role was designed to support the young person to navigate the journey of transition and be able to work in a flexible and more assertive way, to support engagement with Adult mental health services. This may include but not exclusively to: transition from Child and Adolescent mental health services into secondary care mental health services, talking therapies, other community mental health services and community resources that will support the young person's recovery. They will have the ability to provide bespoke time limited recovery focussed interventions with the young person.

Main duties of the job

*To be responsible for delivering bespoke recovery focussed interventions to young people and their families in a way that is integrated with the work of other disciplines within the team and under supervision of a qualified team member.

*To work closely with child and adolescent mental health services and integrate specialist child and adolescent psychiatry with the work of other agencies.

*To provide guidance to other professionals, contributing directly to individual programmes of care.

*Will be a part of the multi-disciplinary team to participate in the evaluation of appropriate interventions.

*To attend Care Planning Meetings and other relevant meetings as required.

*To ensure care is delivered in a person-centred manner ensuring the gender, ethnic, cultural and spiritual needs of service users inform practice.
