Responsibilities

  • Build strong, trusting relationships with young people, encouraging engagement and independence.
  • Provide practical, emotional, and social support to young people transitioning from care to independent living.
  • Conduct assessments of individual needs and contribute to Pathway Plans and support plans.
  • Support young people to identify goals and develop action plans to achieve their aspirations.
  • Provide advice and guidance on accommodation, education, employment, training, health, and wellbeing.
  • Teach and promote essential independent living skills, including budgeting, cooking, and household management.
  • Advocate for young people’s rights and ensure their views and wishes are central to planning and decision-making.
  • Identify and respond appropriately to safeguarding concerns and emerging risks.
  • Maintain regular contact with young people, adapting support plans as circumstances change.
  • Work collaboratively with social workers, education providers, housing services, health professionals, and partner agencies.
  • Assist young people in securing accommodation and maintaining successful tenancies.
  • Promote physical and emotional wellbeing through access to appropriate health and support services.
  • Support young people in managing finances and understanding their entitlements.
  • Maintain accurate, confidential, and timely records of all interventions and support provided.
