Job description

Young People Leaving Care Outreach Support Worker.

Outreach hours are between 8am and 8pm, Monday to Friday.

This is a long-term temporary position, with no fixed end date. If you are subscribed to the update service and have a short notice period, a quick start is possible through a temp-to-permanent placement.

Positions covering Sheffield, Leeds, and Hull.

Experience supporting young people and their families is required.

Full UK Driving License with Business Insurance is essential.

Hours are between 8am and 8pm, Monday to Friday. Flexibility is required to meet the needs of the young people, but the position also offers flexibility for the worker.

Responsibilities include:

  • Completing property inspections
  • Supporting young people to develop independence skills
  • Managing their diary and working closely with permanent staff

Our client is a leading provider of care to children and young people in the area. They are currently recruiting for a Support Worker on a permanent basis.

A quick start is possible if you have a current online DBS.

It is essential that you have experience working within a similar setting and preferably have completed NVQ Level 3 in Children & Young People Workforce or a similar qualification within this sector.

Qualifications/Requirements:

  • A high standard of general education
  • Preferably NVQ Level 3 in Health & Social Care – Children and Young People (CCYP) or equivalent
  • Willingness to undertake further training

Job Purpose:

To be responsible for providing support to vulnerable people (service users) for the company.

Main Duties & Responsibilities:

  1. Assist the young person's allocated social worker in delivering the service, including:
  • Providing transport/escort services as directed
  • Conducting welcome and induction for young people
  • Ongoing assessments of service users’ needs
  • Providing ongoing support and monitoring
  • Assisting with welfare and NHS issues
  • Preparing reports
  • Arranging appointments with partner agencies
  • Maintaining regular contact with service users
  • Informing service users of support groups
  • Assisting with move-on procedures
  • Accurately record, create, maintain, and update electronic records of service users
  • Prepare for and attend meetings as required
  • Provide mediation to service users as needed
  • Use IT packages effectively to support work
  • Work cooperatively across multidisciplinary teams
  • Maintain confidentiality regarding service users, clients, and the company
  • Assist in policy and procedure development
  • Report housing issues and assist in resolving defects
  • Undertake on-call duties as directed
