We work collaboratively withyoung adults throughout their transition from CAMHS to Adult Mental HealthServices. You will be offering support, advice, and information to young adultsas well as family and carers as they progress through to adult mental health services.

We want to ensure that young adultsare supported and empowered to make decisions around their mental health andwellbeing.

The role will ensure thatyoung adults moving from CAMHS services into adult services are sufficientlyprepared and supported to navigate adult provision and networks, whilstmaintaining their progress and promoting independence.

Main duties of the job

The aim of role is to support the successful transition of young adults aged 18-25 into appropriate mental health services and to enhance the social and mental health outcomes of young people experiencing difficulties as they grow into adulthood.

The role will include:

  • Building a therapeutic relationship with the young adult, listening to the young adult fully and helping them to explore their aspirations and concerns about the future
  • Encouragement of responsibility for themselves, their mental health and wellbeing
  • Supporting independence and strengthening their network
  • Helping the young adult to get familiar with adult services (transport, location and people they need to know)
  • Collaborative planning to manage their health and wellbeing

Helping the young adult to find solutions to some of their problems by completing brief interventions.

supporting the young adult, their carers and their CAMHS worker to make their transition run smoothly

  • you will work collaboratively with the lead professional and care coordinator to support the delivery of the transition and care plan
  • alert the care co-ordinator or key worker or allocated worker of any concerns regarding quality, safety or effectiveness of care, or if a young adult has not attended an appointment or there is concern that they are disengaging from services

Job responsibilities

JOB SUMMARY

The post holder will:

Undertake delegated duties under the supervision of registered health care professionals.Consulting as necessary and working collaboratively within the health care teamto deliver therapeutic interventions, psychological and physical care to adefined group of service users.

deliver evidence based planned care,which is safe and effective, in a manner which is caring , compassionate andenables service users to reach an optimum level of functioning, independence, rehabilitationand recovery.

provide care within well establishedpolicies, procedures, protocols, guidelines, and established clinical careplans, whilst acting within the required sphereof clinical competences for the roleat all times.

followingintialassessment by aregistered health care professional, exerciseinitiative and judgement in, care planning, goal setting, delivery,implementation and evaluation of care.

workingcollaboratively and in partnership with others, develop knowledge to promoteholistic care.

demonstratesound knowledge of the careand treatment programmes of individual service users toenable the post holder to contribute and informthe decision making of registered health care professionals.

make appropriatecare adjustments, within the agreed level of autonomy for the post, andbe able to communicate anychanges to service users, carers and the health care team. This may includeregular assessment, evaluation, reportingand escalating relevant information to appropriate health and social careprofessionals.

undertakeenhanced training and preparation to meet the specific clinical needs of theservice and to equip the post holder for an enhanced leadership role.

Experience
  • In providing care and/ or healthrelated procedures(where appropriate to the role);
  • In working in a caring environment at a band 3 or equivalent level of competence;
  • In dealing with the public;

  • In demonstrating initiative, decision-making and prioritisation skills;

Training
  • Must be willing/able to undertake all appropriate Trust mandatory training requirements, and subsequent refresher training relevant to the area.
  • Must be willing/able to undergo job development and training and maintain skills.
  • Demonstrate excellent knowledge of service user safety, eg, Prevention & management of violence & aggression, moving and handling, etc.
Physical Attributes
  • A satisfactory sickness record over previous 2 years (subject to the need to act with fairness and equality of opportunity, particularly where sickness is related to a disability and/or pregnancy).
  • Able to fulfil Occupational Health requirements for the post (with reasonable adjustments if necessary).

Qualifications
  • A-C in Maths/English GCSE or equivalent.
  • Level 5 Apprenticeship/Foundation degree in Health & Social Care or equivalent relevant to the post.
  • Or, level 3 Apprenticeship/Diploma in Health & Social Care with the ability to demonstrate competencies at level 5 and will willingness to work towards achievement of level 5 qualification.
  • Care certificate or required to achieve within 12 weeks of commencement (where relevant to the post).
  • Experience of the speciality area.
  • Willingness to undertake further study/development.
