Job description
Overview

To work as a member of the tissue viability and wound therapy service to undertake the nursing care of patients attending Medway Community Healthcare wound clinics, community nursing and leg ulcer clinics. To provide effective evidence based wound care including the assessment of care needs, the development, implementation and evaluation of plans of care, prescription of treatment and advice to patients who are seen by the service. To support the senior wound therapist in the management of the clinics and take responsibility for allocating workload for short periods in the absence of the senior wound therapist with support. To oversee the healthcare assistants in the team as required and support the senior team members with allocated students and other visitors to the clinic.

Responsibilities
  • Deliver wound care across Medway Community Healthcare wound clinics, community nursing and leg ulcer clinics.
  • Assess care needs and develop, implement and evaluate plans of care; prescribe treatment and provide advice to patients.
  • Support the senior wound therapist in clinic management and allocate workload in their absence.
  • Oversee healthcare assistants and support senior team members with students and visitors to the clinic.

Requirements / Experience

  • Clinical competence in core nursing care provision; evidence of continuing professional development
  • Ability to plan and develop programmes of care/care packages; works within codes of practice and professional guidelines
  • IT literacy; knowledge of wound assessment and Buurtzorg care model; self-care principles
  • Flexibility, adaptability, motivation for community work; open, friendly, team player
  • Ability to make clinically appropriate decisions; strong collaborative working skills; complex problem solving
