In your day to day you will be expected to do the following:

• Conducting assessments to identify the needs of older adults.
• Coordinating care plans and support packages to promote independence.
• Facilitating hospital discharges and ensuring effective transitions to community care.
• Collaborating with healthcare professionals, families, and community services.
• Advocating for service users to ensure their voices are heard and needs are met.
• Maintaining accurate records and adhering to statutory guidelines.
• Supporting individuals in both community and hospital settings.

Requirements:

• Fluency in Welsh (essential).
• Experience in community care and hospital discharge processes (desirable).
• Strong communication, assessment, and organizational skills.
• A commitment to enhancing the well-being of older adults.
