Job description

An opportunity has arisen in the IAA adults team for an enthusiastic and confident individual who can rise to the challenge of a busy front line service. The overall purpose of the well-being coordinator role is to support the department to develop innovative strategies that enable citizens to enjoy more independent lives in their communities. You are required to provide a signposting, information and advice service at the access points of the Adult Social Services to ensure that the department meets its duty within the Social Services and Wellbeing Act 2014.

The IAA team provides a signposting, information and advice service at the access points of the Adult Social Services to ensure that the department meets its duty within the Social Services and Wellbeing Act 2014. The team strives to provide people with a positive customer experience and ensure they are given accurate information, advice, and guidance. We are committed to ensure that the citizens that access our service are getting the right support at the right time by using collaborative communication skills to identify any support needs.

A well-being coordinator is required to carry out proportionate assessments with individuals and their carer’s, identifying personalised outcomes and asset based solutions, whilst applying the national eligibility, taking into account co-productivity and the strengths of the individual and their local community whilst enabling people to live as independently as possible in their own homes.

Knowledge of the Social Services and Wellbeing (Wales) Act 2014 is essential and experience of carrying our strengths-based assessments would be beneficial. The successful applicant would have experience of working in a preventative arena and will be eager to learn new skills and embrace development opportunities. The successful applicant will be expected to work across a number of different sites as the needs of the service dictates.

Educational attainments required for the post include Health & Social care Level 3 qualification or similar qualification within Social Services or a Social Care setting and a willingness to work towards a level 4 is essential. Experience of working with people and their families in a health & social care setting is also essential for this post.

This post is exempt from the Rehabilitation of Offenders Act 1974. A comprehensive

pre-employment screening process will be undertaken which will include an Enhanced Disclosure.

Access to a vehicle for work purposes is a requirement of the role.
