A community health organization in Hereford is looking for a Wellbeing Care Coordinator to support patients in accessing essential well-being services. Your role will involve patient referrals and collaborating with various health professionals to improve community health outcomes. A minimum of GCSEs in English and Mathematics is required, alongside strong interpersonal skills and a commitment to community care. This position offers a full-time schedule of 32 hours within 4 or 5 days.

We’re looking for an enthusiastic Wellbeing Care Co-ordinator to join our friendly team and be the first point of contact for patients needing proactive well-being support across five local practices.
You will work closely with GP teams, Care Coordinators, social prescribers, health professionals, and community groups to help patients get the right support for their needs.
You’ll also be involved in local health projects that focus on helping people who may find it harder to access services.
Working alongside our supportive Wellbeing Team, you’ll help individuals and groups by offering personalised care guiding them to make informed choices that support their health and wellbeing goals.
*This is a position working 32 hours per week across 4 or 5 days*

Key responsibilities will include:
In this varied role, you’ll support patients to access the care and services they need while promoting health and wellbeing across the PCN.
You will coordinate new referrals to the Wellbeing Team, offer advice and signposting, and work one-to-one with patients to help them take an active role in their care. The role also includes supporting group programmes for long-term conditions, engaging in community outreach, and promoting the Wellbeing Team across GP practices and communities. You will work closely with GPs, social prescribers, health coaches, dietitians, and safeguarding leads.
We are looking for a reliable, people-focused person with strong communication skills, able to manage a varied workload, record patient interactions accurately, and collaborate effectively. An understanding of health inequalities/interest in community health initiatives is essential, with a willingness to learn and develop through training and supervision.
