You will be working alongside our GPs and Intermediate care teams, facilitating patient choice, promoting independence and maintaining safety whilst working within your NMC professional code of conduct.

  • Able to use excellent communication skills to communicate at all levels with a variety of people in a professional manner.
  • Act as a role model and provide leadership to a team of staff.
  • Responsible for ensuring high standards of safe and effective care to our patients, working according to Trust policies and within your Code of Practice
  • Able to manage the day to day challenges of ward based working within a sub-acute unit.
  • Work closely within MDT, prioritising safe and effective discharges.
  • Have good organisational skills and be able to prioritise your workload.
  • Be a good decision maker, and able to work calmly under pressure.
  • Manage staff sickness and poor performance.
  • Responsible for supervision of staff and acting as practice assessor to students and new staff.

We are a 16-bedded ward admitting patients from local acute hospitals and directly from their own homes for rehabilitation following orthopaedic, general surgery and medical admissions. We also admit palliative patients for End of life care.

The successful candidate will play an integral part in our MDT, working alongside AHPs and liaising closely with GPs, Intermediate care teams and Community Services.

We are an innovative team, who work collaboratively to facilitate safe and effective discharges for our patients. Our patients often have complex needs and previous knowledge of complex discharge planning would be an advantage.
