We are looking for someone who has significant palliative care experience, can demonstrate excellent communication skills, motivate teams through change and ensure evidence based practice in the field of palliative care.

We support you when you are living with a life-limiting condition, and also help your loved ones and family.We support people aged 18 and over who are living with life-limiting conditions such as cancer, heart failure and lung disease by combining specialist medical care for managing pain and other symptoms with other emotional, practical and spiritual support.We are able to provide care in our inpatient unit and our CNS team are on hand to visit people and provide care for people in their own home.

About the Role

We are looking for someone who has significant palliative care experience, can demonstrate excellent communication skills, motivate teams through change and ensure evidence based practice in the field of palliative care.
In consultation with the Head of Clinical Services, assess potential service users in liaison with Health and/or Social Services commissioners. Ensure that all service users (or when they are not able, relatives) are given the opportunity to participate fully in the care planning process
Responsible for ensuring adequate and appropriate daily staff cover by planning an effective duty rota and participating in the provision of out-of-hours clinical support. Responsible to maintain effective communication and liaison with other departments in the Service and Sue Ryder
To liaise and communicate with other health and social care professionals in Sue Ryder and outside agencies in the planning and delivery of service user care, including junior doctors on placement
To understand and apply a legal policy that supports the identification of vulnerable adults, being aware of statutory vulnerable adult health procedures and local guidance. Furthermore, to assess the impact of policy implementation on care delivery
