A regional healthcare provider is looking for a Band 6 Specialist Nurse for the Virtual Ward. This role involves coordinating care for patients with complex needs, conducting assessments, and developing individualized care plans. You will be part of an integrated healthcare team delivering high-quality service and support across the community. Job summary
An exciting opportunity has arisen to work as a Band 6 Specialist Nurse for the Virtual Ward within the Central Cheshire Integrated Care Partnership Care Community Teams (CCICP). Ensuring the smooth and timely enrolment of patients deemed suitable to receive safe care and treatment in their own home.
You will be part of a team responsible for the care and support of all Virtual Ward patients, with a diagnosis of Frailty, Heart Failure and Respiratory. There will be a high percentage of highly complex patients, so if you are looking to develop your skills and knowledge this is the place for you. You will be joining a team upholding the trust values and behaviours, something the CCICP strongly feel should be embedded. You will ensure consistent delivery of the service and maintain communication and a joined up clinical approach across community and secondary care. As the service develops, the referral sources will be broadened.
You will work under the direction of the Virtual Ward Team leader and to be responsible for assessing and managing the health needs of the identified population and for the development, implementation, and evaluation of programmes of care within a range of target groups.
The Virtual Ward Service is a 7 day a week service (8am-8pm)
Please be aware that a UK driving licence is required for this post, as the role involves driving across the footprint of Mid Cheshire.
Main duties of the jobUndertaking holistic assessment and coordinate the delivery of individualised care as per the multi- disciplinary team.
To work within the agreed framework for Community Nursing Services To assess, provide and review holistic nursing care, advice and information to patients and carers, either by telephone, within the home environment or appropriate care setting in a safe and professional manner using evidence-based knowledge.
Assess, plan, and implement and evaluate a plan of care to meet the patient's physical, social, emotional, and environmental needs, encompassing a rehabilitative/ self-care approach where necessary. Escalate as appropriate.
To provide skilled nursing care and ability to undertake clinical tasks including venepuncture, vital signs monitoring and wound care.
To maintain accurate personal and professional records adhering to MCHFT and Nursing and Midwifery Council policy and guidelines.
To contribute to and co-operate in the co-ordination of off duty and holiday leave with colleagues, ensuring service cover is maintained. Working effectively to support other team members in all aspects of the service.
