A leading healthcare provider in Dartford seeks a Healthcare Assistant to support patients in our Virtual Ward team. You will play a crucial role in remote monitoring and patient care, working closely with nurses to ensure high-quality service delivery. As a Healthcare Assistant, you will be an integral part of the multidisciplinary Virtual Ward team, working under the guidance of a registered nurse to provide essential support in the health and personal care of inpatients within the ward.
A key aspect of your role will involve assisting the Virtual Ward team with remote monitoring, ensuring that necessary equipment is promptly provided and set up for identified patients. You will also ensure patients and their carers are comfortable with the setup. After a patients stay on the Virtual Ward, you will be responsible for collecting, checking, and cleaning the equipment. Your work will help deliver care to a diverse range of patients, including those with life-limiting conditions, patients requiring rapid discharge at end-of-life, and those in crisis who may benefit from preventing unnecessary hospital admissions.

This is a full-time position (37.5 hours per week) based at Livingstone Community Hospital. The service operates seven days a week, from 8 am to 8 pm.
Within this role you will:
To carry out assigned tasks while regularly working without direct supervision involving direct remote care to patients and carers as delegated by the virtual ward charge nurse.
Working very closely with the VW tech provider in ensuring equipment is working well and reporting any concenrns to the VW digital teams help desk.
As part of the VW team, the post holder will provide high quality care to patients, promoting recovery in a manner that maintains patient safety at all times.
The post holder will assist in service and good practice developments.
To assist in providing care as delegated by theSenior Virtual Ward Nurse in chargewithin patients homes.
To maintain knowledge, skills and attitudes necessary to deliver a quality service that is responsive to the changing needs of patients.
To deliver care in accordance with Organisation Policies, procedures and guidelines, according to competencies, as delegated by the Senior Virtual Ward Nurse in charge.
To report any changes in condition of the patient to the Senior Virtual Ward Nurse in charge.

The ideal candidate will have an NVQ Level 3 qualification in care or equivalent experience, along with excellent communication and organisational skills. They will be able to recognise their strengths and work effectively within their scope of competence.
