A community healthcare provider in Ayr is seeking a Community Palliative Care Nurse to manage the Respite and Response Team. This role involves overseeing patient care coordination, triaging cases, and collaborating with multidisciplinary teams to deliver compassionate service. The ideal candidate is a registered nurse with extensive palliative care experience. This position may require some home visits and is mostly virtual.
Job Summary
We are seeking a highly motivated and dynamic professional to join our community team as we continue to enhance and adapt our services to meet the evolving needs of the Ayrshire community.
This position includes line management and operational oversight of the Respite and Response Team, ensuring the delivery of high-quality support services.
The CPCN will also play a key role within the broader community team, overseeing the daily triage of patients on the Respite and Response (R&R) caseload and ensuring the efficient coordination of their care. Where capacity and need allow this role may also extend to the wider community caseload, and, under the delegation of the Specialist Palliative Care Nurses (SPCN), the CPCN may be required to undertake virtual support or domiciliary visits. Furthermore, the CPCN will serve as the primary point of contact for advice and support inquiries directed to the Clinical Admin team.
While this role is primarily virtual, there may be occasions where the CPCN is required to conduct patient assessments in their home or accompany care staff to administer PRN medication before care.
Collaboration is integral to this role, requiring the CPCN to work closely with members of the Multidisciplinary Team to ensure the provision of safe, high-standard, and responsive care for patients who choose to remain in their own homes.
