Job description

North Hampshire Urgent Care, Virtual Care @ Home is recruiting for a Care-Coordinator to work on the service.

Hours: 12 per week working every Monday, Tuesday & Wednesday 09:00 - 13:00

The Virtual Care @ Home Service currently cares for patients with Covid, COPD, Asthma and Acute Respiratory Infections (ARI), supporting and monitoring them at home.

This is an exciting time to join the service as we move to closer working with Hampshire Hospitals Foundation Trusts Virtual Wards, merging the services within the next financial year to create one service caring for the patients within North & Mid Hampshire. This will also facilitate an expansion of the service to other acute condition monitoring.

This role is a fixed term contract until 31st May 2026.

Interviews will be held on the 15th May 2025 at our Head Office location in Farnborough.

Main duties of the job
  • Day to day administration of the VC@H service including liaison with general practice, patients and North Mid Hampshire system wide colleagues.
  • Call patients to explain the service and take relevant history e.g., health conditions, medications, and vaccination status.
  • Contact North Mid practices to arrange collection of our VC@H packs and liaise with the patients with the necessary details.
  • Support the VC@H clinicians in the use of the Whzan platform to onboard and discharge patients.
  • Use AccuRx/GoodSam and set templates to make patients aware that they have come to the end of their monitoring time.
  • Continuously monitor the service email inbox throughout the day.
  • Maintain effective liaison with other agencies and staff concerned with patient care and with all other disciplines with appropriate regard to confidentiality.
  • Ability to use own resourcefulness and common sense, but also a knowledge of when to seek assistance.
  • Liaising with the Operations Manager for reporting and investigating incidents for the service using the Sentinel platform.

TalkPlus provide NHS Talking Therapies, supporting people with common mental health problems in Northeast Hampshire and Farnham place. TalkPlus offers a range of treatments to people who are experiencing symptoms of stress, anxiety, low mood, depression, panic, phobias or OCD. TalkPlus has care pathways to provide timely treatment for perinatal and military veteran referrals.

Hours: 12 per week working every Monday, Tuesday & Wednesday 09:00 - 13:00, with the opportunity to cover additional hours where required.

Role Summary

With support from the wider Same Day Care (SDC) management team, be responsible for the management of the Virtual Care at Home Service (VC@H).

Working autonomously but asking for support where required and recognising where escalation is necessary.

The service in its current form will operate until the end of May 2026.

Job Summary

The Care Coordinator role works closely alongside the nursing team for the Virtual Care at Home service in North Mid Hampshire.

The service manages patients referred in from General Practice, HHFT, OOS, SCAS across the North Mid Hampshire ICP area. Adult patients within an agreed referral criteria are offered admission to the service. Patients are issued with a Pulse Oximeter and onboarded onto the digital platform, Whzan. This allows them to submit their observations three times daily using the WhzApp.

The post holder will undertake key activities across the service, supporting the VC@H nursing team and North Mid practices by providing efficient, timely, well organized administrative and operational support.
