VAWAS Advanced Nurse Practitioner - Hallgarth

Thepost holder is an experienced Nurse with a minimum of two years experienceworking as an autonomous practitioner. They should be working within theirprofessional boundaries and will provide care for the patients within both theirown homes and care homes as part of an integrated team within practices.

Theywill be skilled in initial history taking, clinical assessment, diagnosis,treatment and evaluation of care; whilst demonstrating a safe, criticalthinking clinical decision-making process and expert care delivery for thosepatients who are vulnerable and at risk of avoidable admissions within the generalpractice population.

Thepost holder will be integrated into practice teams, whilst working collaborativelywith the general practice team members, care home staff, patients, and otherHealthcare professionals to meet the needs of this group of vulnerable patients,whilst supporting the delivery of best practice, policy and procedures.

In order to work at this level NMC requirementsfor advanced practice must be met.

Main duties of the job
  • Clinicallyexamine and assess patient needs from a physiological and psychologicalperspective, and plan clinical care accordingly
  • Assess,diagnose, plan, implement and evaluate treatment/interventions and carefor registered patients presenting with an undifferentiated diagnosis andlong-term chronic conditions
  • Assess,diagnosis, plan, implement and evaluate interventions/treatments forpatients with complex needs
  • Proactivelyidentify, diagnose and manage treatment plans for patients at risk ofdeveloping a long-term condition (as appropriate)
  • Diagnoseand manage both acute and chronic conditions, integrating both drug- andnon-drug-based treatment methods into a management plan
  • Prescribeand review medication for therapeutic effectiveness, appropriate topatient needs and in accordance with evidence-based practice andnational and practice protocols, and within scope of practice
  • Work withpatients in order to support compliance with and adherence to prescribedtreatments
  • Provideinformation and advice on prescribed or over-the-counter medication onmedication regimens, side-effects and interactions

Job responsibilities
