We are looking for proactive clinicians to join our urgent response service, providing urgent care to unwell patients in their own home or care homes within 2 hours of a healthcare professional referral. The multi-disciplinary team consists of professionals from different clinical settings, working across Medway to support primary care, community services and the ambulance trust in keeping patients safe and well at home.

Main duties of the job
  • Work as part of the multi-disciplinary urgent response service to support unwell patients in domiciliary and care homes.
  • Operate 0700-1900hrs, 7 days a week; post holders are required to work 12 hour shifts.
  • Rotate between clinical triage function and face-to-face patient consultations.
  • Undertake clinical triage for health care professional referrals into the urgent response service.
  • Undertake face-to-face assessment, diagnosis and treatment for community patients to prevent hospital admission where appropriate.
  • Work as a member of a multidisciplinary team providing clinical care for patients within research and evidence-based guidelines.
  • Support clinical leads in managing cost-effective and efficient practice and actively seek to prevent hospital admissions where appropriate.
Responsibilities and duties
  • Provide care within the organisational policies and guidelines and act as the patient advocate.
  • Communicate complex patient information to various professionals; provide advice and education to patients, families and carers where needed.
  • Liaise with GP practices, social services, voluntary groups and other organisations as necessary.
  • Take informed consent within legal frameworks and carry out assessments for a range of clinical conditions, referencing appropriate colleagues as needed.
  • Identify risk, plan, implement and evaluate care programmes in community settings, including care homes.
  • Maintain records, ensure confidentiality, and submit accurate data for caseload management and service development.
  • Develop and participate in clinical audit, governance activities, and quality improvement initiatives.
  • Participate in training, supervision, appraisals and mandatory professional development; ensure registration is current and compliant with HCPC/NMC where applicable.
Planning, self-development and personal skills
  • Initiate and organise the necessary services to the patient and liaise with other specialist teams.
  • Provide care at home as appropriate and communicate findings to the senior nurse or medical practitioner as required.
  • Participate in self-development programmes and clinical supervision; manage time effectively and adapt to changing service needs.
  • Assess, plan, implement and evaluate care programmes; maintain professional competence through evidence and portfolio development.
Clinical and professional requirements
  • Hold current registration as a Paramedic (HCPC) or as a Nurse (NMC).
  • Experience with clinical triage and community-based care; minor illness management or enhanced clinical assessment qualifications are desirable.
  • Knowledge of community care pathways and integrated working; training in IV therapy and cannulation is preferred or willingness to undertake.
  • Good communication and interpersonal skills; awareness of child and adult protection issues; strong governance and risk management knowledge.
  • Intermediate IT skills; excellent time management; research capability; resilience and assertiveness; understanding of National Service Frameworks related to Minor Illness Management.
