Are you looking for an exciting new role in a high performing and supportive team? Perhaps you are ready for your next challenge? We have a fantastic opportunity for a Registered Nurse or Paramedic to join our West Berkshire Urgent Community Response and Virtual Ward Team.

The post holder will be a key member of our multidisciplinary teams including a Consultant Geriatrician, Advanced Nurse Practitioners, Registered General Nurses, Paramedic Practitioner, and Healthcare Assistants to provide an urgent community response service to patients experiencing a health crisis. The aim is to support and provide virtual ongoing management to them in their own homes (including care homes) rather than hospital admission.

You will be a team player, working collaboratively with staff from all disciplines while maintaining high professional standards. Good communication, effective processes, and procedures must be upheld across the service.

The post involves working 37.5 hours per week, 7 days a week, between 8 am – 8 pm, including weekends.

The role is based at Hillcroft House, Thatcham, with staff parking available on-site.

Key Responsibilities:
  1. Comprehensively assess patients with complex physical, mental, social care needs using advanced clinical examination skills, detailed history taking, biometric monitoring, and evaluation skills.
  2. Undertake new patient assessments and reassessments promptly, reporting back to senior staff for further discussion or assessment.
  3. Analyze signs, symptoms, laboratory tests, and other measures to formulate diagnoses.
  4. Develop individualized treatment and management plans based on evidence-based practices, proactively monitor patients, and review care plans.
  5. Prescribe and install equipment in line with Millbrook-Berkshire Community Equipment Stores competencies, demonstrating correct use to patients if required.
  6. Maintain accurate, up-to-date, and comprehensive records and adhere to reporting procedures.
