Job summary

To be part of the Urgent Community Response Team / Virtual Ward that provides a seven-day-a-week, rapid assessment and intervention for adults of all ages who do not require specialist acute hospital admission but need crisis intervention within a community setting. This may be due to alterations in physical or psychological well-being, or temporary social circumstances that make community maintenance difficult without short-term rehabilitation or intervention.

Virtual wards deliver acute clinical care at home for a short duration as an alternative to hospital care, using a step up/down approach. Patients admitted to a virtual ward have their care reviewed daily by a Geriatrician or Advanced Clinical Practitioner via a digital platform enabling remote monitoring and escalation to a multidisciplinary team.

The post holder will participate in a seven-day working rota, including weekends and public holidays, providing assessments, determining care levels, and initiating care plans to ensure service user safety. The role also involves supporting the virtual element of the service in delivering appropriate care within patients' homes.

Main duties include:

  1. Developing blurred boundary competencies (Nursing / therapy appropriate) to assist the MDT in managing ongoing clinical risks.
  2. Managing referrals into the Urgent Community Response Team/Virtual Ward, liaising and coordinating staff, and managing capacity within the service.
  3. Providing short-term interventions to maintain patients safely at home or prevent re-admission, such as equipment provision or domiciliary care, under Geriatrician-led management.
  4. Working autonomously within Trust policies and professional standards, maintaining accurate and comprehensive records.
  5. Determining clinical diagnoses, developing, delivering, evaluating, and modifying individualized treatment programs, and liaising with other agencies for referral and communication.
