A healthcare organization in England is seeking a Community Physiotherapist to support patients experiencing acute health changes. The role requires excellent assessment skills, experience in community settings, and a relevant physiotherapy qualification. This full-time position offers a salary of £38,682 to £46,580 per annum, with access to a multidisciplinary team and flexible working patterns.
Community Physiotherapist required to join the Shropshire, Telford & Wrekin Urgent Community Response & Virtual Wards service. The post forms part of a seven day service based within one of the Trust's locality teams. The service provides patients who are experiencing an acute change in their health with a safe and effective alternative to hospital admission, supported by a combination of face-to-face visits and digital technology. The post holder will have full access to the Multidisciplinary Team including consultant, GP, Pharmacist, Nurses, Therapists and Health Care Support Workers.

You will need excellent assessment and intervention skills in order to plan, develop and evaluate goal-focused rehabilitation and reablement programs, as well as complex discharge planning experience. An understanding of health and social care provision and the ability to work closely with other organisations and stakeholders is essential. You will need to demonstrate high standards of evidence-based clinical practice. You must have excellent interpersonal and communication skills. You should be able to prioritise your workload, manage your time effectively, and work flexibly within the team. You will be part of a dynamic, developing organisation with well-recognised rehabilitation services providing education, advice, support and rehabilitation for patients.
