Overview

Community Physiotherapist required to join the Shropshire, Telford & Wrekin Urgent Community Response & Virtual Wards service. The post forms part of a seven day service based within one of the Trust's locality teams. The service provides patients who are experiencing an acute change in their health with a safe and effective alternative to hospital admission, supported by a combination of face-to-face visits and digital technology. The post holder will have full access to the Multidisciplinary Team including consultant, GP, Pharmacist, Nurses, Therapists and Health Care Support Workers.

Job Responsibilities

You will need excellent assessment and intervention skills in order to plan, develop and evaluate goal-focused rehabilitation and reablement programs, as well as complex discharge planning experience. An understanding of health and social care provision and the ability to work closely with other organisations and stakeholders is essential. You will need to demonstrate high standards of evidence-based clinical practice. You must have excellent interpersonal and communication skills. You should be able to prioritise your workload, manage your time effectively, and work flexibly within the team. You will be part of a dynamic, developing organisation with well-recognised rehabilitation services providing education, advice, support and rehabilitation for patients.

You will need to demonstrate how you meet our values and behaviours within the job description and person specification. A can-do attitude and positive nature are essential.

Qualifications and Training (Essential)

• Diploma/Degree in Physiotherapy.
• Current registration with HCPC.
• Evidence of specialist Post Graduate training and education with older adults.
• Evidence of core rotations (MSK, orthopaedics, neurology, respiratory, care of the older adult).

Experience (Essential)

• Significant post-graduate experience in a variety of clinical settings with experience in a community setting.
• Experience of working in multi-disciplinary/multi-agency teams.

Skills and Knowledge (Essential)

• Understanding of clinical governance and risk management and their implications for the service.
• Ongoing awareness of recent developments in practice including NICE guidelines.
• Excellent time management skills.
• Excellent interpersonal and communication skills.

• Must be able to meet travel requirements of the post.
