A community health organization in Church Stretton is seeking an experienced Physiotherapist to join their urgent response team. The role requires excellent assessment and intervention skills, focusing on providing care in patients' homes. Candidates should have a Diploma/Degree in Physiotherapy and be registered with HCPC. The organization values a can-do attitude and teamwork. Strong communication skills and experience in community settings are essential.
An exciting opportunity has arisen to join the Shropshire, Telford & Wrekin Urgent Community Response & Virtual Wards service as a Community Physiotherapist. The post will form part of a seven day service based within one of the Trust's locality teams. The service provides patients who are experiencing an acute change in their health a safe and effective alternative to hospital admission, supported by a mixture of face-to-face visits and digital technology.
We are looking for a Physiotherapist who shares our passion for providing excellent quality of care and can help deliver a recommended and agreed treatment/care plan to service users in their own home. The post holder will have full access to the Multidisciplinary Team including consultant, GP, Pharmacist, Nurses, Therapists, and Health Care Support Workers.
You will need excellent assessment and intervention skills in order to plan, develop and evaluate goal focused rehabilitation and reablement programs, as well as complex discharge planning experience. An understanding of Health and Social care provision and ability to work closely with other organisations and stakeholders is essential.
You will need to demonstrate high standards of evidence based clinical practice. You must have excellent interpersonal and communication skills. You should be able to prioritise your workload, manage your time effectively, and work flexibly within the team.
We are looking for enthusiastic, motivated, and conscientious & experienced Physios to work with the established therapy teams. We are a dynamic, developing organisation with well recognised rehabilitation services providing education, advice, support and rehabilitation for patients.
