Do you want to help deliver care to people in their homes in the Inner London Boroughs of Kensington and Chelsea, Westminster, and Hammersmith & Fulham, with an opportunity to work with a supportive, skilled team and develop yourself to provide admission avoidance care?

Make a difference to those who require rapid response in their own homes. We are looking for Band 6 Registered Nurses, Paramedics, or Therapists with experience in community or tertiary services who want to continue developing as independent practitioners. (Newly Qualified staff cannot be considered for this role).

Please note: Interviews will be face-to-face only.

Main duties of the job

We aim to recruit passionate, proactive, and committed team players who will provide excellent care in patients' homes. Community experience is not essential, but applicants must demonstrate the Trust’s core values and understand working in patients' homes, working alone, and being flexible with hours, including weekends, public holidays, and face-to-face visits.

This position involves visiting various locations throughout the London Boroughs of H&F, K&C, and Westminster. Our service operates seven days a week, from 8 am to 10 pm, 365 days a year.

Detailed job description and main responsibilities

  • Assess, plan, implement, and evaluate healthcare needs in collaboration with patients, carers, and the multidisciplinary team.
  • Work within your designated areas of responsibility as part of the team.
  • Provide nursing care to patients in the community.
  • Manage patients with potentially undifferentiated diagnoses, including assessment, treatment, follow-up, and care planning.
  • Ensure all patients on your caseload are managed effectively and discharged safely, either to their GP or for further assessment, rehabilitation, or ongoing care.
  • Stay updated on current issues in Primary and Secondary Healthcare.
  • Liaise with community service partners.
  • Be an active member of the team and contribute to its development.

You must hold appropriate UK professional registration. This role is subject to the Rehabilitation of Offenders Act 1974 (Exceptions) Order 1975 (Amendment) 2020, and a disclosure check will be required through the Disclosure and Barring Service.
