Job description

Urgent Community Response Nurse/Paramedic

To assess, plan, implement, and evaluate healthcare needs in collaboration with patients, their carers, and other members of the multidisciplinary team.

Responsibilities include:

  1. Providing nursing care to patients in the community.
  2. Assessing patients with potentially undifferentiated diagnoses through objective and physical assessments, treatment, follow-up, and developing care plans.
  3. Ensuring all patients on the caseload are managed effectively and discharged safely to their GP or for further assessment, rehabilitation, or ongoing care.
  4. Staying updated on current issues and developments in Primary and Secondary Healthcare.
  5. Collaborating with community service partners.
  6. Being an active team member and contributing to team development.
