Job description

City Health Care Partnership CIC is seeking highly motivated and enthusiastic practitioners to join their team at the Urgent Treatment Centres across Hull and the East Riding of Yorkshire.

The practitioner will be responsible for clinical assessments, including history-taking, examinations, diagnosis, treatment, or onward referrals for patients of all ages. Consultations will be conducted face-to-face, via telephone, or digital platforms. Patient presentations may include undiagnosed illnesses, minor injuries, acute issues, and exacerbations of long-term conditions.

The role requires ensuring the highest standards of clinical care, acting within professional boundaries, and supporting policies and procedures. The practitioner will serve as a positive role model, supporting junior staff, and working flexibly between urgent and primary care to deliver safe, expert care.

Effective leadership, motivation, dedication, and enthusiasm are essential. The successful candidate will collaborate with partner organizations to provide seamless, timely care, promoting evidence-based practice, audits, and adherence to current guidelines and research.

The role involves working across all bases within the Hull and East Riding locality, benefiting from a multidisciplinary team and exposure to diverse healthcare settings. Ongoing training and development will be supported through in-house competencies and university courses when available.
