Job summary

Primary Care 24 is recruiting for a clinically focused triage role based at Wavertree Technology Park, with hybrid provision for salaried staff. Salaried, part-time and bank positions are available, with shifts Mon-Fri 18:00-midnight and 24/7 weekend coverage.

You will oversee patients awaiting contact, coordinate with leadership, and ensure safe care within defined waiting times. The role requires extensive emergency health care experience and a degree in Clinical Practice, including telephony

Qualifications

• Extensive experience in emergency health care settings.
• Significant expertise in clinical triage.
• Minimum 3 years post-qualification experience.
• Clinical Examination and Diagnostics Level 6/7 or relevant telephony triage experience.
• Experience in urgent care/primary care or similar setting.

Responsibilities

• Liaising with medical/nursing leadership to oversee patients awaiting contact.
• Coordinate with Clinical Coordinator/Shift Manager to ensure safe care within waiting times.
• Escalate concerns to Clinical Coordinator/on-call teams.
• Review patients awaiting contact to minimise clinical risk and signpost appropriately.
• Triage with red-flag approach, prioritising high-risk patients (elderly, palliative, deterioration risk).
• Apply clinical skills and reasoning to transfer patients between dispositions and signpost when required.
• Provide skilled assessment via telephone consultation using clinical judgment and algorithms.

Job description

Location: Wavertree Technology Park, with hybrid available for salaried team.

Contract: Full, Part time & Bank positions available.

Shifts: Monday‑Friday 6-hour shifts 18:00‑midnight & 24/7 service over the weekend.

What will I be doing?

• Liaising with nominated medical / nursing leadership team to provide oversight and coordination of patients awaiting contact.
• Managing clinical demands in collaboration with the Clinical Coordinator / Shift Manager to ensure delivery of safe care to agreed waiting times.
• Escalating concerns / priority patients to the Clinical Coordinator / On‑call teams.
• Reviewing patients awaiting contact to provide oversight and minimise clinical risk by actively filtering for potential signposting to more appropriate services.
• Based around a Red Flag approach of triage, prioritise patients in line with the service escalation plans which include the elderly, palliative patients and those who have a risk of deterioration.
• Apply clinical skills and reasoning to ensure patients are transferred safely between different clinical dispositions and signposted when required.
• Providing skilled and effective assessment of patients presenting clinical need, through telephone consultation, using professional clinical judgement with the support of clinically based algorithms.

Salaried: £49,583.00 per annum (pro rata). Home working available once completed probationary period. 27 days annual leave pro rata – previous NHS Service will be taken into account.
