Job description

City Health Care Partnership CIC are looking for highly motivated, enthusiastic individuals to join their team of practitioners in the Urgent Treatment Centres based across Hull and the East Riding of Yorkshire.

Role Responsibilities
  1. Clinical assessment of patients, including history-taking, clinical examination, diagnosis, and treatment or onward referral.
  2. Conducting urgent and primary care consultations via face-to-face, telephone, or digital platforms.
  3. Managing patient presentations such as undiagnosed illnesses, minor injuries, acute conditions, and exacerbations of long-term illnesses.
  4. Ensuring the highest standard of clinical care and supporting the delivery of policies and procedures.
  5. Collaborating with partner organizations to provide seamless and timely care, enhancing patient experience.
  6. Working across various bases within the Hull and East Riding locality.
  7. Working within a multidisciplinary team and supporting the new multi-professional clinical model.
Candidate Requirements
  • Experience caring for and treating patients with minor injuries and/or illnesses.
  • Ability to act within professional boundaries and demonstrate effective leadership skills.
  • Motivated, dedicated, and enthusiastic attitude.
