Job description

Urgent Care Services provides short-term health, therapy, and care support to enable service users to remain in the community or home environment. This may include treating acutely unwell individuals who would otherwise require hospital admission and supporting hospital discharges. The service operates 24 hours a day, 365 days a year.

Night care includes end-of-life care, catheter care, and other urgent, unplanned care that cannot wait until the following day. This care is delivered by a dedicated night nursing team.

Home is nationally evidenced as the best place for recovery and regaining independence. Therefore, enablement and rehabilitation are the primary focus, with active participation from service users to achieve maximum independence.

At KCHFT, we collaborate with East Kent Hospitals University Trust, which is developing a virtual ward for patients with acute respiratory infections, such as chest infections, or underlying conditions like COPD, to be cared for at home.

Patients who agree to earlier hospital discharge will be monitored at home using technology such as apps, wearables, or other medical devices remotely. They will be supported by consultants, specialist GPs, or advanced practitioners. The specific technology used is still being scoped, as we co-design the virtual wards with patients and colleagues in the coming months.

Key Responsibilities:
  1. Assessment, planning, implementation, and evaluation of holistic care.
  2. Developing integrated care plans in partnership with patients.
  3. Ordering equipment and making onward referrals as necessary.
  4. Acting as a lead and senior practitioner to support junior staff in assessments and care pathways for patients with complex needs.
  5. Working as part of a multidisciplinary team to provide high-quality patient care.
  6. Performing clinical interventions and identifying patient deterioration, taking appropriate action.
  7. Managing a caseload of patients with complex needs daily.
  8. Supporting in duty roles and completing triages.
  9. Assisting the Clinical Lead in team management.
  10. Providing recommendations to senior management to prioritize efforts, drive service improvements, and meet statutory requirements.

Rated ‘outstanding’ by the CQC, we are committed to delivering high standards of patient care and excellent services to improve community health. We seek outstanding individuals who share our values: compassionate, aspirational, responsive, and excellent.

We believe that equity, diversity, and inclusion enhance patient care and workforce satisfaction. We welcome applications from individuals of diverse backgrounds, perspectives, and experiences.

Our application process includes collecting information about your protected characteristics, which is confidential and used to promote diversity and equitable access to opportunities. Applicants may opt for the guaranteed interview scheme if applicable due to disability.

We support flexible working arrangements to suit personal and family commitments. Discuss your preferences with us, and we will aim to accommodate them.
