Kent Community Health NHS Foundation Trust is looking for outstanding individuals to provide high-quality patient care in a dedicated, compassionate environment. The role focuses on holistic assessment, care planning, and teamwork to support individuals with complex needs while promoting diversity and equity in access to health services.
Urgent Care Services provides short-term health, therapy, and care support to enable service users to remain in the community or home environment. This may include treating acutely unwell individuals who would otherwise require hospital admission and supporting hospital discharges. The service operates 24 hours a day, 365 days a year.
Night care includes end-of-life care, catheter care, and other urgent, unplanned care that cannot wait until the following day. This care is delivered by a dedicated night nursing team.
Home is nationally evidenced as the best place for recovery and regaining independence. Therefore, enablement and rehabilitation are the primary focus, with active participation from service users to achieve maximum independence.
At KCHFT, we collaborate with East Kent Hospitals University Trust, which is developing a virtual ward for patients with acute respiratory infections, such as chest infections, or underlying conditions like COPD, to be cared for at home.
Patients who agree to earlier hospital discharge will be monitored at home using technology such as apps, wearables, or other medical devices remotely. They will be supported by consultants, specialist GPs, or advanced practitioners. The specific technology used is still being scoped, as we co-design the virtual wards with patients and colleagues in the coming months.
