Job description

The Crisis Resolution and Home Treatment team is a multi-disciplinary team offering intensive short-term crisis intervention and home treatment in Chichester and Bognor Regis. Its aims are to prevent hospital admissions and to facilitate supported discharges from hospital.

This post offers the Trust's £2,000 Golden Hello or Relocation Incentive for Band 5-6 Nurses & Practitioners, (pro rata and subject to terms and conditions; please see attached Protocol Document for full details).

Main duties of the job

We are looking to recruit someone, who is enthusiastic, with excellent assessment, risk management and communication skills and is committed to working as part of a team and in partnership with service users and carers.

You will need to have some experience of crisis resolution practice / acute care and be willing to work flexible hours to enable the team to provide a 7 day service.

You will receive regular supervision and have the opportunity for personal and professional development as well as contributing to the team's development.

You will be expected to travel across sites in order to cover an urban / rural area.

Job responsibilities

This is an exciting and dynamic role that offers the opportunity for development and implementation of high-quality services. Previous experience of working in acute services or home treatment teams is desirable, although not essential. We offer excellent staff support through clinical supervision and teamwork.

Crisis Resolution Home Treatment Team is integral to our Urgent Care Pathway and ensure people presenting to acute mental health services receive a compassionate, person- centred assessment.

This is a seven day per week service, which may necessitate some working unsocial hours, however, working unsocial hours does bring a salary increase, in line with Agenda for Change guidelines.

Qualifications
  • Mental health nurse, Occupational therapist or social worker with current registration [mental health/LD/Adult], appropriate to the job role.
  • Completion of Mentorship Course/ENB equivalent
  • Evidence of continuous professional development
Knowledge
  • Experience of working alongside people with complex health problems in the designated care group
  • Experience of supervising staff
  • Experience of providing a range of clinical interventions to people in the designated care group with a variety of health problems
  • Experience of working consultatively with professionals
  • Experience of partnership working within and across various statutory and non statutory teams and agencies
  • Experience of working within the CPA process
  • Previous experience of working in the community
