Job summary

Nottingham City General Practice Alliance is seeking a General Practitioner to lead on the On the Day Service within the Primary Care Network. You will deliver patient-centred, urgent care with timely consultations, advice, treatment and referrals in a fast-paced primary care setting.

You will work with a multidisciplinary team to provide seamless care, contribute to service development and governance, and ensure high-quality patient outcomes. Right to work in the UK is required.

Qualifications

• GMC Registration with the General Medical Council.
• Current UK professional registration required.
• Right to work in the UK at the time of application.

Responsibilities

• Making professional, autonomous decisions in relation to presenting problems.
• Assessing the health care needs of patients with undifferentiated and undiagnosed problems.
• Undertake detailed clinical assessments, including history-taking, physical examinations, and ordering diagnostic investigations when clinically appropriate.
• Screening patients for disease risk factors and early signs of illness.
• Formulate and implement evidence-based management plans tailored to each patient's clinical needs, ensuring safe and effective prescribing practices.
• Manage medical emergencies within the scope of primary care, escalating or referring patients to secondary or specialist care services as necessary.
• Develop care plans for health in consultation with patients, in line with current disease management protocols.
• Provide counselling and health education; refer to other care providers as appropriate.
• Record clear and contemporaneous consultation notes to agreed standards; collect data for audit and QOF requirements.
