Job summary

gtd healthcare, one of the largest NHS commissioned primary and urgent care providers in the North West, seeks a Care Co-ordinator at Royal Preston Hospital and Chorley Hospital. You will register patients and direct them to the most appropriate service, contributing to a 24/7 care co-ordination operation.

You will work with clinicians and the NHS 111 service, handle appointments, maintain records in clinical systems, and support a responsive, patient–focused service while upholding information

Qualifications

• Basic reception or call handling experience in a health-setting.
• Experience recording data into clinical systems and directing clients appropriately.
• Familiarity with NHS processes and patient confidentiality requirements.

Responsibilities

• Deliver a high-quality care co-ordination service to patients and healthcare professionals.
• Accurately record data across clinical systems for each contact and direct appropriately.
• Book appointments across multiple services within agreed timescales.
• Liaise with the Senior Care Co-ordinator and clinical lead to maintain smooth patient flow.
