Epsom Twilight Team are a friendly and supportive small community team. And we are are looking for a caring, passionate and motivated health care assistant/s willing to continue to expand and develop their own skills and knowledge while supporting the patients and the team. The role is a community role so driving is essential. All nursing visits are delivered in pairs. The working hours are 6:30pm - 11:30am we currently have 5 shifts available however, we would consider someone willing to work a minimum of 2 shifts per week.
Main duties of the job
Working under the direct supervision and direction of a Registered Nurse to support and assist the nursing team to provide a high quality standard of care and the promotion of physical, mental and emotional health to individual patients in their own homes.
To be an effective member of the Multidisciplinary Team carrying out assigned tasks involving direct care in the support of the senior team member. The post holder is responsible for their own practice, working within the defined limits of competence and within the scope of the role. The registered nurse retains accountability for the care delivered by the HCA.

Clinical
Detailed job description and main responsibilities
- To support and assist the senior team member in the provision of a high standard of care and the promotion of physical, mental and emotional health to individuals and client groups.
- To be an effective member of the Multidisciplinary Team carrying out assigned tasks involving direct care in the support of the senior team member
- Undertake clinical interventions as delegated by a senior team member
Health Promotion
- Participate in the assessment process of care needs and implementation of programmes of holistic care within set standards
- To be involved in supporting, educating and informing family/carers where appropriate
- Assist in the promotion of health and wellbeing of patients
Administration And Clerical Duties
- Maintain clear and comprehensive, signed and contemporaneous records according to SDHC procedures
- Take and record telephone messages, respond and liaise appropriately
- Manage stationery and documentation supplies
- To be responsible for maintaining data entry and other record keeping e.g. recording mileage and annual leave on staff databases
Team Working
- Work as part of the team participating in procedures and duties in accordance with agreed standards relevant to role
- Assist with clerical and administrative duties as required
- Maintain regular contact with the senior team member and communicate all relevant issue

Person Specification Qualifications Essential Criteria
- Basic education to GCSE (or equivalent level) incl English and Maths
Desirable criteria
- Evidence of professional development.
- NVQ
Experience Essential criteria
- Experience relevant to particular post

- Previous Health/Social experience.
Skills Essential criteria
- Ability to work independently following spoken or written instructions.
- Ability to work effectively as part of a team, valuing contributions from team members
