Are you looking for a new and exciting challenge? This is a great opportunity to join our Integrated Single Point of Access Team. We are seeking experienced and motivated clinicians with a nursing qualification and an interest or experience in triage.
The post holder will work at a group level to integrate the triage team and support service delivery. This role follows a review as part of our transformation programme and involves a multidisciplinary workforce within the ISPA, alongside an experienced administrative team.
You will be responsible for triaging and prioritising all new referrals, including urgent community response and hospital-at-home referrals. Expert clinical knowledge and judgement are essential for effective decision-making.
This office-based role includes some clinical time, with flexible working options to support your work-life balance.
The role involves using clinical knowledge and risk assessment skills to prioritise referrals through the triage service, ensuring care is prioritised based on urgency and supporting patient pathways.
You will collaborate closely with primary and secondary care, as well as local third sector and voluntary organisations, to ensure patients receive appropriate care. The role also involves virtual consultations with staff, patients, carers, and other professionals.
The post holder must ensure all contacts with the clinical triage service lead to effective and appropriate patient outcomes and be adaptable to changing organisational and community needs.
