Job summary

Home Support Matters in the United Kingdom is seeking a Triage Care Worker to provide short-term care during peak demand periods. You will support new customers from day one of their journey, building rapport and ensuring high standards of care.

Qualifications

• Experience in care or support roles.
• Confident assessing needs and making informed decisions.
• Excellent communication and calm under pressure.
• Solid understanding of safeguarding and risk.
• Proactive, organized and professional approach.
• Full UK Driving Licence and access to own vehicle with business insurance.

Responsibilities

• Provide short-term care and support, focusing on new customer integration.
• Build strong rapport with customers and their families from the outset.
• Identify risks, safeguarding concerns and any required interventions.
• Communicate clearly with branch teams, field care supervisors and other professionals.
• Ensure accurate, timely documentation that meets regulatory standards.
• Represent Home Support Matters with professionalism, empathy and expertise.
