Job summary

Wokingham Borough Council is recruiting a Qualified Practitioner to join the Adult Social Care Team on a full‑time, permanent basis. The role focuses on transitional safeguarding within the PACT team, supporting young adults with complex needs and safeguarding risks in a trauma‑informed, strengths‑based way.

You will build trust, assess needs, and work with partners to reduce risks and promote safer, more stable futures for service users while contributing to exemplary council performance.

Qualifications

• Experience supporting young adults with safeguarding needs.
• Posts in adult or children's social care with relevant exposure.
• Strong relationship-building and communication skills, especially with reluctant service users.

Responsibilities

• Build trusting relationships with young adults who may disengage from services.
• Assess needs and develop practical, person-centred support and risk management plans.
• Identify and manage safeguarding risks including exploitation and homelessness.
• Support safeguarding work including Mental Capacity assessments and Section 42 inquiries where appropriate.
• Coordinate with other services to provide joined-up support for safety and independence.
