Job summary

Somerset Council is recruiting a Leaving Care Worker to support young people aged 16 and over who are in care or recently left care. You will work closely with each young person and professionals to assess needs and plan a path toward independence.

The role involves personal advising from age 18, trauma-informed practice, and frequent travel to homes and supported accommodation across Somerset. You will join a wider team, benefit from reflective supervision, and enjoy flexible, hybrid working

Qualifications

• Experience working with looked-after children and care leavers.
• Knowledge of safeguarding and path planning processes.
• Ability to work with multiple agencies and stakeholders.

Responsibilities

• Support care leavers aged 16–25 and develop Pathway Plans.
• Travel to homes and supported accommodation across Somerset.
• Collaborate with social workers and multi-agency colleagues.
• Provide trauma-informed, person-centred support.
