A leading social work agency is seeking a Qualified Social Worker to join the Transitions Team in Swansea. This role involves working with young people aged 17-25 with learning disabilities, conducting assessments, and planning transitions. The position offers hybrid working arrangements and requires a minimum of 6 months' experience in the UK, along with relevant qualifications and registration.

A registered social worker is required to join SwanseaSocial Servicesto work with young people between the ages of17 – 25 in transition and general learning disabilities over 25 yrs old.Ideally we are looking for someone who can offer full time hours ideally Monday-Friday37 hours a week, for an initial3 month contract.
This role offers hybrid working with home visits required plus office working circa 3 days per week. Further discussions can be held about this direct with the team manager.
The work will involve carrying out statutory assessments and reviews under the Social Services Wellbeing Act 2014; planning for young people’s transition from child to adult services and arranging suitable provisions to meet their needs. You will need to effectively manage a complex case load. Additionally, this posts includes assessing risk and supporting people to manage these risks. Experience of working with people with a learning disability, autism is essential and transition to adulthood work is desirable.
