Salary: Grade H. Scale 29 - £38,626 to Scale 34 - £43,693 with a professional bar at SCP 31 £40,476**

Hours Per Week: 36hours

Base Location: Wilshaw House, Wilshaw Lane, Ashton-under-Lyne, OL7 9QG / Jubilee Gardens, Gardenfold Way, Droylsden, M43 7XU

The Role

We are looking for a Transitions Social Worker to be based within Childrens and Adult Services in Tameside.

Your work will include screening and advising on referrals for Children with Disabilities that are preparing for adulthood and working with colleagues in both Children’s and Adult Services, as part of busy and supportive teams. As part of your role you will be managing a caseload, undertaking Care Act (2014) Assessments, creating Support Plans, undertaking work in relation to safeguarding, Mental Capacity Assessments and Best Interest Decisions, and signposting to internal and external services where necessary to enhance wellbeing and independence. You will be working in accordance with the SEND Code of Practice 2014 and the Children and Families Act 2014.

You will be Social Work qualified and have professional registration with Social Work England. You will have excellent communication skills and have the ability to build effective professional relationships with other disciplines and with other local health and social care services to enable a coordinated response to individuals and families.
