Transition Worker - Clinical/Counselling Psychologist

Join our team to enhance awareness of transition issues, challenges, and available services for young people. Foster excellent working relationships between Children and Adolescent Mental Health Services (CAMHS) and Adult Mental Health Services (AMHS).

Responsibilities include:

1. Promoting mental health wellbeing and physical outcomes.
2. Supporting stability in education and employment for young people.
3. Reducing episodes of crisis.
4. Implementing an agreed job plan with specified activity and throughput levels.
5. Coordinating care for children, young people, and families during their transition process.
6. Managing risk within team, Directorate, and Trust structures.
7. Working within relevant professional, ethical guidelines, and policies.
8. Contributing to the review and update of transition protocols between CAMHS and AMHS.
9. Participating in screening and outcome measures to demonstrate treatment effectiveness and gather user feedback.
