Transition Support Worker [Tameside Met BC]

Pay rate: £19.99 per hour

Vitalis is recruiting for a Transition Support Worker to work in a Staying Close Project in Tameside.

Tameside Children’s Services are seeking committed Transition Support Workers to join the Staying Close project. This pilot service supports children who have left care and previously lived in children’s homes, providing practical, employment, emotional, and social assistance as they transition out of care.

The aim is to equip young people with skills for a successful and positive life once they move into their own homes. Support continues until the age of 21.
