A healthcare organization in Dalton seeks a dedicated professional to support young people with learning disabilities as they transition to adult services. The role includes managing a caseload, delivering health workshops, and liaising with various stakeholders to promote smooth transitions. Candidates should ideally have a Level 3 qualification and a passion for helping others. This position offers an opportunity to gain a Level 3 apprenticeship alongside practical experience.
To support young people with learning disabilities as they transition from children to adult services. Predominantly working with people age 17 ½ and above. The role will involve working closely with education providers, health professionals, families, and carers to promote smooth, person-centred transitions. The post-holder will provide education and support around health needs, capacity and consent, and annual health checks, and act as a key link between services.
If you don't already have a Level 3 qualification or equivalent experience in a relevant subject, RDaSH are offering this post alongside the opportunity for you to complete a Level 3 apprenticeship qualification relevant to your role. As an apprentice within RDaSH you will gain experience, learn work-related skills and obtain all the tools needed to thrive. An apprenticeship includes regular training with a college or other training organisation. Around 20% of your working hours will be spent training or studying, leading to a nationally recognised qualification.
