A healthcare organization in Doncaster is looking for a Transition Support Worker to assist young people with learning disabilities in their transition from children's to adult services. You will liaise with families and professionals, promote independence, and provide tailored support. The role includes the chance to achieve a Level 3 apprenticeship qualification, gaining practical experience in a caring environment. This position provides an excellent opportunity to contribute positively to the lives of individuals during a critical transition phase.

To support young people with learning disabilities as they transition from children to adult services. Predominantly working with people age 17 1/2 and above
The role will involve working closely with education providers, health professionals, families, and carers to promote smooth, person-centred transitions.
The post-holder will provide education and support around health needs, capacity and consent, and annual health checks, and act as a key link between services.
If you don't already have a Level 3 qualification or equivalent experience in a relevant subject, RDaSH are offering this post alongside the opportunity for you to complete a Level 3 apprenticeship qualification [relevant to your role].
As an apprentice within RDaSH you will be given the opportunity to gain experience, learn work-related skills and obtain all the tools needed to thrive.
An apprenticeship includes regular training with a college or other training organisation. Around 20% of your working hours will be spent training or studying. This leads to a nationally recognised qualification.
