A community health service provider in Doncaster is seeking a Transition Support Worker to assist young individuals with learning disabilities transitioning to adult services. The role involves providing essential support and liaising with educational and health professionals. Applicants should possess level 2 qualifications in English and Maths, and previous caring experience is valued. This position includes an opportunity for a Level 3 apprenticeship qualification.

To support young people with learning disabilities as they transition from children to adult services. Predominantly working with people age 17 1/2 and above
The role will involve working closely with education providers, health professionals, families, and carers to promote smooth, person-centred transitions.
The post-holder will provide education and support around health needs, capacity and consent, and annual health checks, and act as a key link between services.
If you don't already have a Level 3 qualification or equivalent experience in a relevant subject, RDaSH are offering this post alongside the opportunity for you to complete a Level 3 apprenticeship qualification [relevant to your role].
As an apprentice within RDaSH you will be given the opportunity to gain experience, learn work-related skills and obtain all the tools needed to thrive.
An apprenticeship includes regular training with a college or other training organisation. Around 20% of your working hours will be spent training or studying. This leads to a nationally recognised qualification.
