The Hillingdon Young People's Transitional Service is offered to young people, with health needs and/or complex health needs who are preparing to leave children services. The Transition Nurse Specialist for children's community team role is to facilitate, support and co-ordinate the transition from Children's to transition to Adult Health Services. The role includes development of the transition service, participation in strategic expansion and collaborative working across health agencies , including Hillingdon Hospital , social care and education. The role includes a large educational component educating both Children's and Adult Services to facilitate the transition process.
To take the lead in coordinating the transition process for young people with health and or complex health needs or life limiting conditions.
To compliment the current service provision within the Children's Community Nursing Team Service and to work closely with the other members of the Transition.
To assess referrals to the Transition Service and where necessary direct referrer to a more appropriate service.
To ensure that a Health Summary/Health Passport is produced, in agreement with the young person, with identified health actions/health plan to be completed within a set time scale.
To assess the physical, social and psychological health and well-being needs of identified young people giving information, advice or counselling and support and making referrals as appropriate.
To provide specific advice, training and support to staff in health and other partner agencies who are providing care to young people in Hillingdon
We will offer clinical supervision, regular appraisal and support in achieving your personal development plan as well as peer support from colleagues in similar roles.
You will have the ability to provide case management of an identified caseload of children and young people with complex health needs by utilising and promoting evidence-based care.
You should have excellent interpersonal skills to support you in managing complex discussions with a range of agencies and individuals, including care providers, patients and families.
In addition the ability to work in partnership with parents and multi-disciplinary agencies to provide skilled care to children and young people within the community to support safe transitioning to adult health services.
You will be highly organised and have a flexible approach to work, able to deal with difficult situations autonomously with demonstrable, excellent communication skills.
This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions.
