Job summary

NHS Jobs in Wembley is seeking a Transformation Lead to drive the Primary Care Single Offer and commissioning delivery. You will translate strategy into borough level plans, track milestones, risks and benefits, and support PCNs to embed quality and performance feedback.

You will lead projects with clear deliverables, timelines and KPIs, ensuring each initiative has a logic model and a theory of change underpinning it.

Qualifications

• Managing transformation programmes aligned to the boroughs priorities (e.g. out-of-hospital care affecting the left shift)
• Milestone tracking, maintaining risk logs and tracking benefits realisation
• Lead the implementation of projects, ensuring that key milestones are achieved and that the required outcomes are met.
• Ensure that each project that the postholder leads on has a logic model and a theory of change underpinning it, as well as having clear deliverables, timescales and KPIs

Responsibilities
