Are you an experienced healthcare professional with extensive knowledge of the transfer of care process? Do you thrive in a leadership role where you can make a real impact on patient flow and integrated care?

We are seeking a dedicated and dynamic Transfer of Care Team Lead to oversee and manage the Transfer of Care Team at The Princess Alexandra Hospital NHS Trust (PAHT). This role is crucial in ensuring safe, timely, and efficient patient transitions from hospital to community care settings.

Main duties of the job

As Transfer of Care Team Lead, you will:

  1. Oversee the daily operations of the Transfer of Care Team, ensuring a seamless discharge process.
  2. Work closely with the Transfer of Care Hub Lead within the Care Coordination Centre to optimise patient flow.
  3. Foster strong relationships and maintain clear communication with internal stakeholders and external system partners across West Essex and East & North Hertfordshire.
  4. Ensure accurate data collection and reporting to support system-wide collaboration and decision-making.
  5. Lead and support a team that works across multiple hospital sites, ensuring best practices in discharge planning and care transition.

To be successful in this role, you will need:

  1. Extensive experience in the transfer of care process within a healthcare setting.
  2. A professional background in nursing, allied health professions (AHP), or social work.
  3. Proven leadership and team management skills.
  4. Excellent interpersonal and communication skills to engage effectively with multidisciplinary teams and external partners.
  5. Strong analytical and problem-solving abilities to enhance service efficiency and patient outcomes.
