Job summary
This role is key in developing the operating processes for the Transfer of Care Hub (ToCH), and the relationships with health and care partners across the acute, community, social and voluntary care sectors to drive the changes required to streamline the discharge processes across all discharge pathways.
The Transfer of Care Hub (ToCH) will be based at St Helier Hospital and will bring together key specialist teams from across the health and social care sector under one umbrella to facilitate discharges of patients with ongoing complex health and care needs.
The ToCH is undergoing significant transformation to enable a fully integrated model joining up providers to deliver effective discharge planning and pathway management, and to optimise the current resource of discharge coordinators currently based within the acute trust.
Sutton Alliance have developed a clear vision to improve and streamline the discharge processes for patients leaving hospital after an acute spell of care, and this role will be responsible for setting up the ToCH across the health and care system and will start this exciting programme towards enabling this vision.
The successful candidate will need an in-depth knowledge of discharge processes aligned with the ability to forge excellent working relationships with operational and managerial partners across the health and care system.
Main duties of the jobThe Transfer of Care Hub (ToCH), based at St. Helier Hospital, brings together key specialist teams from across the health and social care sector under one umbrella to facilitate discharges of patients with ongoing complex health and care needs. These include a team of community health providers, South West London Integrated Care Board (SWLICB), London Borough of Sutton Social Care and Brokerage teams (LBS), housing services and voluntary organisations.
The post holder will lead the strategic development of the TOC as set out and agreed by the Sutton Alliance. They will implement and deliver responsive out of hospital pathways and provision to ensure that collaborative services within the community (notably those provided by Sutton Health and Care, LBS, Primary Care and the voluntary sector) are as responsive as they possibly can be to the capacity demands within the acute trust.
The post holder will be accountable for the operational management of the ToCH including direct line management of the restructured Discharge Coordinator Team Leaders and matrix management of all other professionals who are part of the infrastructure.
