Exciting Opportunity! We're seeking an enthusiastic, proactive Integrated Discharge Hub Lead to join our team from 1st September 2025 for 8 months to cover maternity leave.
This senior role involves overseeing the strategic and operational management of patient discharges from acute settings to community providers, ensuring a smooth process. You will collaborate with our Health and Social Care Senior Operational Leads to enhance discharge procedures.
Your key responsibilities include:
- Optimising patients' length of stay to facilitate timely discharge.
- Developing and implementing systems for coordinated working with partner agencies.
- Evaluating and redesigning discharge pathways in line with current guidance.
- Monitoring and reporting on discharge performance to foster continuous improvement.
- Leading the Trust's discharge standards and policy adherence.
You will also support Trust-wide projects, collaborating with NHS and non-NHS partners.

Main Duties
Operational Management / Leadership
- Lead the delivery of the Hospital Discharge Hub, promoting admission avoidance and safe, timely discharges.
- Manage discharge hub teams, ensuring high standards of practice and patient care.
- Contribute to clinical quality and standards development.
- Develop systems to accurately record performance and reduce unnecessary delays.
- Report performance metrics to the CCG in compliance with legislative requirements.
- Develop, implement, and review discharge-related policies.

Education & Qualifications
- RGN/Midwife or BSc/Diploma in AHP professions.
- Knowledge of discharge models such as discharge to assess.
- Experience in partnership working and delivering care.

- Understanding of hospital, community, and social care sectors.
- Knowledge of national policies in discharge planning.
- Master's degree or working towards it.

Salary: £61,927 to £68,676 per annum, excluding HCAS.
