Transfer of Care Discharge Hub Lead (Band 7 or 8a depending on experience) Maternity Leave 8-months fixed term cover.

Exciting Opportunity! We're seeking an enthusiastic, proactive Integrated Discharge Hub Lead to join our team from 1st September 2025 for 8 months to cover maternity leave.

This senior role involves overseeing the strategic and operational management of patient discharges from acute settings to community providers, ensuring a smooth process. You will collaborate with our Health and Social Care Senior Operational Leads to enhance discharge procedures.

Your key responsibilities include:

  1. Optimising patients' length of stay to facilitate timely discharge.
  2. Developing and implementing systems for coordinated working with partner agencies.
  3. Evaluating and redesigning discharge pathways in line with current guidance.
  4. Monitoring and reporting on discharge performance to foster continuous improvement.
  5. Leading the Trust's discharge standards and policy adherence.

You will also support Trust-wide projects, collaborating with NHS and non-NHS partners.

Main Duties

Operational Management / Leadership

  1. Lead the delivery of the Hospital Discharge Hub, promoting admission avoidance and safe, timely discharges.
  2. Manage discharge hub teams, ensuring high standards of practice and patient care.
  3. Contribute to clinical quality and standards development.
  4. Develop systems to accurately record performance and reduce unnecessary delays.
  5. Report performance metrics to the CCG in compliance with legislative requirements.
  6. Develop, implement, and review discharge-related policies.

Additional Information

This role is subject to a Disclosure and Barring Service check due to the Rehabilitation of Offenders Act 1975.

Salary: £61,927 to £68,676 per annum, excluding HCAS.
